Identification for enrolling adult. Acceptable forms of identification are:

- Any valid state or federal government issued photo id

- A Georgia Driver’s License, even if expired

- Valid employee photo id

- Valid U.S. Passport id

Proof of Residency:

- Notarized Affidavit of Enrollment or Affidavit of Legal Residence (if parent/guardian does not own or lease the

property in which they reside on a full time basis)

- Mortgage statement, warranty deed, quit-claim deed, or security deed in the name of the parent/guardian

ü Birth Certificate

ü Records from previous school ( include school withdrawal form discipline report, last report card, test scores and name and address of previous school

ü Social Security Card for student
